Including bathing, grooming, dressing and assistance with mobility.
Light maintenance housekeeping, laundry and meal assistance.
Including support to engage in social activities and companionship, assistance to appointments and shopping.
Physiotherapy, occupational therapy, podiatry and registered nurses.
Qualified staff to support in monitoring and assisting you to administer medications safely.
Equipment to promote independence and improve mobility and access.
Minor modifications to the home to improve safety and access within the home, including installation of grab rails and ramps.
Minor garden maintenance to ensure safe access to your yard.
Individualised transport options to access the community or to attend appointments independently.
A short term support to allow your carer time to take a break.
Company at home or out to share moments with and speak to.
Case management and Coordination of Supports to ensure that individual needs are being met.
You may be eligible for supplements to your package, meaning that you can access additional funds to support your care needs. These include Dementia and Cognition Supplement, Oxygen Supplement, Veterans Supplement, Enteral Feeding Supplement and a Viability Supplement.
Your Care Coordinator will determine if you are eligible and assist you to access this additional funding.
